• Student Permission/Waiver - October 1, 2009
• WI Dells Track Waiver - Bring to Event

Below are the Fall-2008 Wisconsin Dells Raceway WEEVA Registration forms. They are printable PDF documents that should be returned by their respective due dates to WEEVA.

WEEVA has tried as much as possible to eliminate the quantity of Forms each team/student needs to fill out each year, however, there are some forms that can not be eliminated or combined due to safety and insurance purposes.

Notes: If students under 18 do not bring their track waivers to the challenge, they will NOT be allowed to enter the racetrack!

Notes: If your team has a problem where these dates are difficult or impossible to reach, contact WEEVA to explain and make arrangements. If you do NOT contact WEEVA ahead of time, a $10.00 late fee will be charged to your team per item that is received AFTER the deadlines above!
